You will need to write down your scores during your conferences with your
homeroom teacher in the provided spaces above. If you have reached a Meets
OR Proficiency score in any of the following required assessments, you will need
to color in that box. This will indicate that you have reached, or exceeded your 5 th
grade goal. On the following sheet, you will write down what you and your
teacher discussed on how to IMPROVE and reach the MEETS OR PROFICIENCY
level!

write down YOUR personal goals for each assessment in the beginning of the year!
As a Luella Lion, we know that you will reach your 5th grade goal with NO
PROBLEM! So push yourself to the NEXT level by setting a goal for each
assessment BEYOND the 5th grade goal. You may set these goals by yourself, or
with your parents or guardians! Remember there is no limit to what you can
achieve!

Tell me Your Goals!
Here at Luella, we make sure that our students reach their
academic goals for the year. We also know how important you all
as individuals are important to us. So we would like to know what
your personal goals are for this year as well! Say for instance that

you play soccer, and your goal for the year is to score at LEAST
two goals in a game. So you will write that down as your goal,
then you will write down the steps that you think you will need to
take in order to reach this goal.
Once you have met this goal, then we will encourage you to
set another goal to surpass your prior goal! So with that being
said, take some time to write down at LEAST three personal
goals that you would like to make in regards to yourself as an
individual on the next page.
Remember, this is your opportunity to reflect on three or
more things that you will love to accomplish this school year! As
your teacher, I will try my best to make sure that you reach these
goals, as well as your academic goals set by yourself and Luella.
