    Respected Sir,
    This is with reference to HR circular No:HRD/45/2014 dated 11-12-2014 to announce
    an opportunity for employees to adopt SR 1961 or to remain fall under SR 2005. In
    the context, undersigned want to bring some facts into your kind notice and
    consequently seeking approval to adopt SR-2005
    Undersigned was on EX-Pakistan leave w.e.f 12-12-2014 to 11-01-2015 to perform
    Umrah.
    Prescribed due date to choice the option was expired before my rejoining on 11-012015.
    After joining, undersigned was unable to concentrate fully and fall short to inform
    the above fact to management due to the regular visits to hospital foe medical
    treatment of my son.
    Competent authority is requested to consider my application and allow my
    conversation from SR 1961 to SR 2005. Copy of relieving/ joining report and reports
    medical visit is attached for your kind perusal.
