    TENANT’S 90 DAY NOTICE TO VACATE

    Hanover Park, IL. 60133

    According to the terms of my lease requiring a 90 day notice, you are hereby advised of my intent to
    vacate the premises on or before this date 10/01/2010.

    I will be cleaning the unit as to leave it in a good condition. Please send me any specific move out
    cleaning instructions, if you have any. I will remove my personal belongings and turn in my keys on or
    before 10/01/2010.

    I would appreciate the return of my security deposit within (5) FIVE days after I have vacated my unit. The
    forwarding address for the return of my security deposit is:
