Team Binder Table of Contents

(This form must come first in your three-ring binder)

1) Team Roster

2) Trading Card Pockets for Player & Coaches Cards

3) Coaching License(s) & Risk Management Card(s)

• If you have not obtained your Coaching License you must include a dated and signed letter of
intent stating that you intend to obtain your license within the one year grace period.

4) Coaches Code of Ethics

• To be signed and updated EACH YEAR by the head coach, assistant coach and team manager
• 1X1 (no larger) photos of coach, assistant coach and manager should be attached by paper clip.
Please identify photo with name on back.

5) Membership & Medical Release Forms / Birth Certificates / 1X1 Player Photos
• MUST be updated each Fall Season (a new form can be filled out or the original form can be
signed and dated by parent acknowledging all information is current.)
• Birth Certificate for each player
• 1X1 (no larger) player photos should be attached, by paper clip, to corresponding Membership
and Medical Release Form. Please identify photo with name on back.
