COMMENTS ACTION
18 Men on site today. The signing in book is not being kept up to date operatives sign in but M
don’t seem to sign out.
Item 1: The scaffold had been loaded out which meant that no one could access the M

scaffold as it was covered in Rubble and could possibly be over loaded.

Item 2: In the basement there was a set of broken steps I brought this to Gary attention
A
and he advised me they were going in the skip the next day.
Item 4: On the ground floor there was lot of openings in the floor in the rear area. This can
cause trips and falls. Gary was shown and would action. B

Item 7: The stairs from the basement were covered in Rubble bags which would and can
cause problems with safe access and also blocking fire route. Gary was advised of this and A
told me again this was going away in the morning. When working like this all access and fire
routes must always be kept clear.
Item 14: I brought it to Gary’s attention the fire bells were not working he advised me there A
were new ones that he would install.
Item 20 and 21: Tool box talks and safety meetings is something that both KPM and
NEXUS need to look at as for tool box talks Amanda Upton will resume the tool box talks
under her remit.
