For the duration of her stay in the Philippines, our child will be staying with above name (Maria Angeles and Maria Lourdes de Guzman will reside in the Philippines) at this address: 896 Makinabang Baliuag, Bulacan Philippines 3006 This Letter of Consent is being issued this 30th day of June 2011 at (Union City, CA) in compliance with immigration requirements for minors traveling to the Philippines not accompanied by or joining either parent.
