AFFIDAVIT OF CLARIFICATION

I, AMADOR ORIFORN CUDAPAS, married, of legal age, natural born Filipino

citizen, and a resident of Brgy. Foz, Dingras, Ilocos Norte, Philippines, after having been
duly sworn to in accordance with law hereby depose and states that;

1. I am the biological father of JASON ANTHONY CUDAPAS who was born on

March 06, 1986 in London, United Kingdom;

2. Vitally, my son Jason and CHERYL CLAIRE CASSANDRA COULDWELL started to

engage into a romantic relationship in the year 2005;

3. In the course of their relationship, Jason and Cheryl had a son named NYE
LIDDEN ADORE COULDWELL CUDAPAS who was born on October 30, 2007 in
London, United Kingdom;

4. Notably, after the birth of Nye, my son Jason and his partner Cheryl mutually
decided to end their relationship and separated for good;

5. Several years, thereafter, my son Jason got married to MICHELLE RAMIREZ.

Consequentially, from that time on, Michelle has been using the name
MICHELLE RAMIREZ CUDAPAS in all of her dealings, either public or private;

6. During their wedlock, Jason and Michelle begot a child named NYCO ADORE
RAMIREZ CUDAPAS who was born on June 25, 2020 in San Nicolas, Ilocos
Norte, Philippines;

7. At present, my grandson Nyco is under my care and custody in Brgy. Foz,

Dingras, Ilocos Norte, Philippines;

8. Anent the foregoing, NYE LIDDEN ADORE COULDWELL CUDAPAS and NYCO
ADORE RAMIREZ CUDAPAS, pursuant to the Family Code of the Philippines,
are lawfully considered as half-brothers, their father being one and the same,
who is JASON ANTHONY CUDAPAS;

9. Relatively, Nyco is in no way related to Nye’s biological mother, CHERYL

CLAIRE CASSANDRA COULDWELL, either by consanguinity or affinity,
considering that the former’s blood lineage only runs through his biological
parents Jason and Michelle and never from Cheryl;
 10. I am executing this affidavit to attest to the veracity of my statements and for
whatever intents and purposes it may lawfully serve.
