Checklist HR Departure The Manager of the departing employee is responsible to ensure that all items on this checklist are

e completed as part of the standard departure procedure. Once completed the form is to be filed with the employees personnel record.

Notice sent to all staff announcing departure Notice sent to Customers announcing departure, if appropriate Accounting advised of last day to close out pay account Key(s) collected o Access card collected Email id disabled/email rerouted All login passwords disabled Company ID card, if possessing one, collected Company equipment (laptop, printers, etc) collected Desk cleaned out Active files/projects passed on Phone greeting reset/changed; password provided to Manager Company paid services, if any, terminated (eg home phone lines, Gym memberships) Company credit cards, taxi chits, etc collected Employee reminded of post employment obligations regarding confidentiality, IP, non-compete. Box provided for personal effects
