    Step 1 Fund Raising Indicate your choice for food item/s you want to sell. If you dont want to sell food, go to Step 2 and indicate the activity you want to do to raise money for your organization. Food or Beverage Item give a name and detailed description of item/s. Example A desserts; various home baked brownies, pies, cookies Example B Frito pie; (Fritos covered with chili, lettuce, tomatoes, cheese and salsa) Example C sodas assorted can drinks First Choice: Second Choice: A Cupcake Bar-Decorate your own cupcake or buy ready decorated Baked Goods-Desserts and Cookies

    Activity give a name and detailed description Example D tie-dye t-shirts; Participants will purchase a t-shirt and receive assistance with dying the shirt. Example E carnival game, ring toss participants will get 3 tries at throwing plastic rings around various bottles and receive prizes for successfully tossing 2/3 rings around the bottles designated with various prizes. Prizes include packaged candy, mood rings, and stuffed animals. First Choice: Second Choice:

    For food items only circle which electrical implements you will use Crock pots Deep Fryers Steamers other (please describe) Step 5: Ice -- if you intend to use ice, you must make arrangements with the cafeteria as soon as possible. Do not wait until the day of the event as ice supply is limited. SLR will not supply coolers.
