Hosss Community Night Katy Miller Memorial Fund

The Miller Family is holding a Community Night in honor of Katy Miller. This Community Night will be held at Hosss at the Indiana Location on July 20th, 2013. The money that is raised will benefit a family in need within the community. We hope to see you there!!

If you decide to come to the community night, here are some things you might want to know:
The community night will be held all day at the Indiana Hosss on July 20th, 2013. This will not be valid on the day before or the day after or any other day that is not July 20th, 2013. You will need to take only the second page of this packet to Hosss with you on July 20th, 2013. You will present the paper to the greeter when ordering. If this paper is not presented, the money will not be given to the Memorial Fund. If there are going to be separate checks in your party, you will need one of the papers (second page of the packet) per check. 20% of each check that is taken that day (that has the paper attached), will go to the Katy Miller Memorial Fund. If the second page is not presented at Hosss, the money will not be given to the Memorial Fund. You must have the paper for this to be valid. As many of you know, we also have a Katy Miller Golf Scramble every summer to raise money for a family in need. The money that is raised from this community night will be combined with the money that is raised from the golf scramble and given to a family in need. All of this money will be donated to a 13 year old girl and her family. The little girl is suffering from the same disease Katy had, PSC, and is on the transplant list. We would really like to help her and her family as much as we can get through this terrible crisis for many of us know how hard it can be.

*To qualify for the 20% payout, our group must have a minimum of $300.00 in total pre-tax food and beverage sales. **To qualify for the 25% payout, our group must have a minimum of $1,200.00 in total pre-tax food and beverage sales. This applies to a one-day, single location, pre-scheduled event. The purchase of gift cards is not included as part of your organizations community fundraising totals.
