Terms of Reference March 2009

Aim

This partnership aims to improve the wider health and wellbeing by increasing awareness of and access to local services. This will be
achieved by bringing together a wide range of local providers with the Hounslow Jamia Mosque and other local faith centres for the
delivery of partnership events, projects and services onsite.

The partnership will focus on the population in and around the central and southern Hounslow wards and northern Whitton ward
however it is recognised that others working in or visiting the area may benefit.
Objectives

The objectives for the partnership include:

1 Deliver a programme of events and projects covering a broad range of health and
wellbeing issues based on the key priorities that are identified for this population.
2 Identify and progress an annual work programme which outlines details of events
and projects that will be delivered each year.
3 Establish and monitor progress of Events, projects and services, forming working
sub-groups when necessary.
4 Identify partnership resources available for development of projects and events
and submit joint bids for external funds where possible.
5 Communicate the achievements of the partnership with regards to projects,
events and onsite services delivered through Council, PCT and other
communications teams.
Performance Measures

The following measures will be used to evaluate the partnerships performance based upon its objectives set out above.

5. Achievements communicated -Annual report produced on events, projects and

-Media coverage of partnership events and
projects.

Reporting Requirements
The partnership will report to the Health and Social Care Partnership Board every 6 months on progress, key achievements and areas
where higher level assistance would be of benefit.

Frequency, Structure and Recording of Meetings


• The meetings will be a maximum of 2 hours and occur 3 / 4 times per year
• Meetings will be chaired by ?.
• Minutes will be taken and circulated by the Public Health Team.

Group Membership
Membership includes a wide range of organisations and providers whose services relate to the wider determinants of health and well-
being in Hounslow. The list is not static and can be added to following agreement by the group. Group members are asked to attend
when possible or send a representative.

group information updates. These are people who may have attended in the past as a representative or who have requested to be
kept informed. They may wish to attend when topics relevant to their service area are raised on the agenda but attendance will not be
expected at each meeting.
