General Remarks :
1

As on date of inspection the work is completed. The field officers are not informed the progress of work
they are informed only after completion of the work. Hence the field officers are requested to inform the p
work in future stage wise for effective Q C inspections at regular intervals.

The exact location of patch was not recorded in M Book and hence it could not possible to verify the
below top layer.

Depths are not verified to avoid further damages to the road as the patch work is of scattered natu
compared on area basis.

The Patchwork surface finishings is not done as per specification.

B T edges of patch work are disturbed.

The patch work done area at some places disturbed. The field officers are requested to notify the contra
disturbed patch work and see that it should be rectified immidiatly.

High berms are existing near patch work area. Hence The field officers are requested to immidiatly remo
berms to avoid stagnation of water.

The quentity of BT patch work for 40mm could not be assessed as it is covered with 12mm BT patch work

The field officers have executed WMM/12mm patch work and 40mm/12mm patch work. At the time of ins
work is completed, hence the surface areas of patch work done with 12mm is only verified.

The Executive Engineer is requested to effect the recovery of Rs.27 675/- from contractor's bills to
quentity in 12mm patch work and intimate the recovry particulars such as MB No. and CB Voucher No. t
in the compliance report.
Note :

The Q C R furnished above are not basis for making payment on the work as it does not purport to be covering all the aspects of
field Engineers are requested to check up the work and see that the work is executed to specified standards and satisfy them
making payment.

The field officers are requested to check the recovery calculations as per agreement conditions before effecting the recovery.

The recovery proposed is as per the E-in - C(R&B) (Admn & Roads) circular Memo No. 12582/vigl/1/2005-1 dated. 17.10.2006. T
Engineer is requested to keeping view regarding recovery to be effected.

The recovery amount proposed in the QCR does not include the item of work for which rectification is required as pointed o
remarks. The Q C R issued based on the observations made on the day of inspection and if any reveling or damages occur
subsequently should be watch carefully and proper action shall be taken for rectification. After rectification only payment shall be
solely based on this report.

The Q C R furnished above are not valid for any evidence in the court of law.
