I.Instructional Competence(70%)
A. Instructional Competence (70%)
1. Objectives formulated in my plan met all the indicators.
2. Four of the indicators were considered in the selection of the content and the preparation of
instructional materials for lessons.
3. Indicators a, b and e were considered in selecting teaching strategies.
4. Four indicators were met in relating new lesson with previous knowledge.
5. Four indicators were considered in providing appropriate motivation.
6. Four indicators were met in presenting and developing lessons.
7. Indicators a, b, d, and e were met in conveying ideas.
8. Indicators a, b, and c were met in utilizing the art of questioning to develop higher level of
thinking.
9. Four indicators were met in ensuring students participation.
10. Indicators a, b, c and d were considered in addressing individual differences.
11. Indicators a, c, d and e were considered to manifest mastery of the subject matter.
12. 1. Indicators c and d were used to diagnose learners needs.
2. Three of the indicators were used to evaluate learning.
3. At least 90% passed in Grade 8-Silver and Grade 8-Nickel and achieved the desired outcome
of the lessons.
13. 1. Maintenance of the classroom was ensured most of the time.
2. Four of the indicators were met in maintaining a classroom conducive for learning.
B. Learners Achievement (20%)
1. Achievement level in the post test improved by 2.5% in Grade 8-Silver and Grade 8-Nickel
over pretest.
C. School, Home and Community Involvement(5%)
1. 70%-90% of schools programs/plans/policies were disseminated to schools clientele.
2. Served as a member of the community in the implementation of its projects.
II. Professional and Personal Characteristics (20%)
1. Acted on needs/requests in accordance with the prescribed rules and regulations and accepted
norms of conduct and behavior always.
2. Demonstrated truthfulness, candidness, uprightness and freedom from deceit most of the time.
3. Rendered service over and above the regular functions and even beyond the regular time more
often than not.
4. Showed initiative and resourcefulness when needed but not often.
5. Showed polite, kind, and thoughtful behavior toward the public/clientele in manner of speech and
actions.
6. Integrated concern for people at work, office and clientele and supervisor-subordinate relationship
into work situation.
7. Influenced and developed confidence in co-workers.
8. Showed stability of performance under pressure when needed.
9. Conformed to usual principles of law; was just and unbiased most of the time.
10. Dressed in accordance with proper decorum and prescribed rules and neat in appearance always.
III. Punctuality and Attendance (10%)
1. Had incurred 1 undertime/tardy during the semester.
2. Had incurred and applied 1-day absence during the semester.
