Introduction

1. There are 4 Worksheets (Instruction, Template, Reference, and List) composing this ICT Inventor

2. The Instruction worksheet provides the introduction information and the instructions on how to u
this particular ICT Inventory file. It also includes the Name(s) of responders, their office, and the da
accomplished.

3. The Template worksheet is where you provide the required data and information for the ICT Asse
Inventory.

4. The Reference worksheet is your reference for better understanding the Template worksheet. It
provides descriptions, clarifications, examples, and other related information relevant to accomplis
the Template worksheet.

5. The List worksheet is a protected worksheet which contains the necessary data for data validatio
purposes on the Template worksheet. You DONT need to read this worksheet.

Instruction
1. Go to the Template worksheet and observe/familiarize the column headers.

2. Go to the Reference worksheet and read through each itemized row corresponding to the column
headers of the Template worksheet. Make sure you understand the description, information guide, a
remarks entries.
3. Start completing the Template worksheet.

4. Once finished, GO BACK to the Instruction template and complete the accomplishment form (top
of this worksheet). Provide the NAME(S) of personnel who accomplished the Template worksheet, O
and DATE ACCOMPLISHED.

Itemized list of ICT assets deemed significant for GIFMIS


readiness

Select ONE (1) from the given drop down list. Please note that it is imperative to
separate desktop computers from being a computer set to the following: 1.) desktop
computer is composed of the CPU box, keyboard and mouse & 2.) monitor. Monitors
has to be listed as a separate article or item in the list.

Provide the serial number of the ICT asset as found in the device or property settings.

Contains the property number assigned by your office to

Example: ACER, HP, IBM, etc.
Provide the specific model type and/or number of the ICT asset. Example: Aspire One
(Acer), Presario CQ62 (Compaq), Thinkcenter M57 (Lenovo), etc. This particular
information is usually found in the back portion of the device alongside the barcode
sticker for the device.

If template is not altered, this field will automatically be generated


based on the selected Article item.
If Manufacturer is not known (i.e. LAN cables), you may put 'Not
Known' in this field.
If Model is not known (i.e. LAN cables), you may put 'Not Known' in
this field. You may also refer to the ICT Asset Identification User
Guide for sample images on locating this particular information.
If Serial Number is not known (i.e. LAN cables), you may put 'Not
Known' in this field. You may also refer to the ICT Asset
Identification User Guide for sample images on locating this
particular information.

If the ICT asset is provided by the LGU, Congressman, Senator,


local donor, foreign project, or any other external source, you may
choose 'Asset is Donated' option and provide specific donor in the
Remarks column. If asset is provided by a service provider such as
a modem for the internet connection, you may use 'Asset is
Leased' option and provide the specific service provider in the
Remarks column. However, if the fund source for the ICT asset is
not known such as the network cables used for the Local Area
Network (LAN), then you may choose 'Not Known' option and
provide additional information in the Remarks column such as
when was it installed in the office, what's the main purpose of it,
etc.

Source of Fund

Itemized list of fund source for acquiring the ICT asset.

Select ONE (1) from DepEd CO, DepEd RO, DepEd DO, Asset is Donated, Asset is
Leased, or Not Known.

Date of Acquisition

Contains the date of acquisition for the ICT asset.

Default format for this column is month/day/year. Example if
