AFFIDAVIT OF CORRECTION
I, MARK LOIS PENA, of legal age, Filipino citizen, and resident #49 Manuela
Ave., Dona Subd., Pamplona Tres, Las Pinas City, after having been sworn to in
accordance with law, hereby depose and say, - that:
On September 5, 2014, I purchased a ticket from Air Asia flight from Macau
to Manila in the amount of PHP 810.00 MOP, for the passenger MAYVILLE PENA,
scheduled trip on September 11, 2014, and a confirmation reference/Booking no.
V17B6U was mailed to me thru my email address at loispenad@yahoo.com
Upon printing the said ticket, I was surprised that the name of passenger
was not my sister instead my name appears therein as the passenger.
That I gave all the details correctly thru online transaction when I purchased
the said ticket, but, inadvertently my name was recorded as the passenger instead
of my sister MAYVILLE PENA.
I now desire and request the Air Asia to kindly change the name of
passenger from MARK LOIS PENA to MAYVILLE PENA for scheduled trip on
September 11, 2014 via Air Asia.
I am executing this affidavit to attest to the truth of all the foregoing facts
and for all its legal intents, effects, consequences and purposes.
