Greetings!
This letter is with regard to the incident which occurred last July 12, 2016.
That on or about 2:40am, a patient was brought to the CT Scan Department for a
procedure on her right ankle. I was one of the attending Radiologic Technologist that
time and admittedly, because of the number of patients that we have to attend to and
because most of the procedures that night were that of a Brain CT, I mistakenly
performed a CT Scan of the brain instead of the one ordered by the attending physician.
The following morning, the patient was brought back to the CT Scan Department so
that the proper procedure can be done. However when I reported back to work, one of
our radiology resident, Dr. Amoroso, complained and asked for an explanation for the
said miscue.
I respectfully answered her question, gave her my reason why it had happened and
even asked for an apology for my mistake. But instead of accepting my apology, she
insisted that I was at fault and made an incident report wherein I was accused of
insubordination. In addition, she also ordered me to make my own incident report.
With that, I am inclined to answer the allegations made in her incident report.
First of all, I never disrespected Dr. Amoroso nor have I undermined her authority. As
our Radiology Resident, I have utmost respect towards her and by no means do I intend
to malign her. What bothered me is that she accused me of insubordination when in fact
all I did was answer her question.
An act of insubordination refers to the disobedience or defiance of a lawful order made
by a superior a person of authority. In the case at hand, I never defied nor disobeyed any
orders from my superiors. What I did was to merely answer her question, and to
reiterate, I even admitted to my mistake and asked for an apology. All of it was an
honest mistake and no harm was done to the patient.
With that, I now respectfully submit this incident report before your office, not to evade
responsibility but to tell my side of the story.
