in Tata Consultancy Services Ltd. (TCSL) from __________ to________ and have ceased to be an
employee of TCS Ltd. effective _________.
I undertake that I will be transferring my Provident Fund (PF) accumulation / Pension to my new employer
and will submit Form 13 to my new employer for forwarding the same to TCSL or I will be withdrawing my
Provident Fund (PF) accumulation as permitted under the Employees Provident Fund and Miscellaneous
provisions Act, 1952 (EPF & MP Act).
I have been duly informed by the HR team of Tata Consultancy Services Ltd. about the statutory provision
under EPF & MP Act that in the event TCSL does not receive my PF transfer application or PF withdrawal
application within 36 months from my date of release, my PF accumulation with TCS EPF Trust shall be
transferred to an account to be called the Inoperative Account and no interest will be credited to my PF
account.
