Dear Dir. Lakshmi:

Greetings from School of Teacher Education and Liberal Arts!
My students who are currently enrolled in Educ 114A (Basic Research Proposal) are conducting
a research entitled, The effects of combined extracts of Azadirachta indica and Psidium
guajava on thrombocyte induced white mouse (Mus Musculus) that aims to determine the
plants effects on increasing the blood platelet of patients with positive cases of dengue. We
would like to request the number of patients diagnosed with dengue reported in your office
(Baguio City and Benguet) for the last three years. The data would be used to substantiate the
analysis of the research being conducted.
In anticipation of your favorable response, thank you very much.
Very truly yours,

Prof. Wilfred B. Bambico, PhD
