Title: Develop Clinical Portfolio (2000 words)

Instructions:
You are required to develop your own clinical portfolio.
Your portfolio must consist the following details:
1 Resume
2 College Transcripts
3 1 page essay on describing your personal value and belief on the nursing profession
4 Outline your plan for professional development with supporting documentation of activities
and learning outcome
5 Health records which include immunization and current physical condition
6 List of volunteer position with supporting documentation
7 List of professional organization you are in supporting documentation
8 Your job description
9 Performance evaluation by peer or manager
10 Competency check list
11 Copy of your nursing licence/ Certificate with renewal dates
12 Professional conference attended with supporting documentation

Guidelines:
An electronic copy of your Clinical Portfolio assignment must be presented using Microsoft Word,
Arial text font size 11 with 1 spacing between lines.
Academic misconduct (e.g. plagiarism, collusion) is a serious offence therefore work must
be original and supported by references to scholarly materials.
Using APA 6th ed. referencing system, references cited in the body of the text must match
those in the end reference list.
Marks will be deducted for grammar and spelling errors.
