DESCRIPTION: I assisted the site council by running this meeting. The topic was a
presentation of the Bergman Blast before and after school program presented by the Blast
Coordinator followed by the Bergman Principal discussing state assessment results. It is
important for teacher leaders to collaborate with families and community. I feel this
activity meets ISLLC Standard 4 because this site council meeting builds relationships
with families and community partners. I also feel that standard 1 was being addressed
because site council was providing information about state assessment results to build a
unified vision with the PLC.

DESCRIPTION: The daycare providers indicated concerns about a child in their care
earlier in the day. They had developmental questions. They wanted to know information
about identifying Autism and the characteristics of Autism. I met with them in the
afternoon to provide information about spotting the signs of Autism. I also provided a
referral tablet that our department gives to the medical community to make referrals to
our department. I also spoke with the director of Kinder Care about organizing hearing-
vision and developmental screenings to the children at the daycare who are birth to 3. I
believe this opportunity represents ISLLC Standard 4 because it is building relationships
with community partners.

presented at the workshop. I provided important information that might be helpful to

leader helps to promoted professional growth of the teaching team.

DESCRIPTION: During Team meeting, I presented a language data form that I created
to help establish baseline and show progress of children with language delays. I
explained how to use and what information you can obtain by using it. I answered

DESCRIPTION: I contacted and scheduled for Nancy with OCCK, the assistive
technology lending library, to meet a family that I service. The goal was to introduce the
family with a variety of equipment choices so that they may find equipment that will be
helpful for their childs learning and functioning in their home. We also hope the family
will identify equipment they may want to purchasing using the Deaf/Blind Grant funds.
This activity supports ISLLC Standard 2 because is promotes the use of the most
effective and appropriate technologies to support teaching and learning as well as ISLLC
Standard 3 because it helps to obtain technological resources.

DESCRIPTION: I scheduled and conducted a transition conference with the preschool
team and a family that I service. The goal was for the family and preschool team to meet
each other and to establish the steps that need to be taken to determine if this child
qualifies for preschool service once he turns 3. This supports ISLLC Standard 1 because
it provides opportunity for the team to build a vision as well as Standard 4 to help with
collaboration between the preschool team, infant toddler team and the family.

DESCRIPTION: I met with the 2-year old room teachers to review goals of the child
that is in their care. I also wanted to find out what goals they wanted to address for this

community childcare facility as well as Standard 2 because I am reviewing the IFSP
goals with the teachers to ensure better instruction for this child.

DESCRIPTION: I met with the 2-year old room teachers to review goals of another
child that is in their care. This particular child has extensive needs as he demonstrates
behaviors that are related to autism. The goals for this child are engagement and
participation. I needed to find out what the teachers can do while working with 13 other
children and what goals they want to target to make teaching him easier. This supports
ISLLC Standard 4 because I am collaborating with the community childcare facility as
well as Standard 2 because I am reviewing the IFSP goals with the teachers to ensure
better instruction for this child.

DESCRIPTION: I spent a week in Omaha, Nebraska taking a class so that I could take
the test to get my certification for Lactation Counselor. I provided a breastfeeding
training to my colleagues so they may provide breastfeeding support to the families that

training.

DESCRIPTION: I am having Nancy with OCCK, the assistive technology lending
library, meet with me and the family I work with about every 4-6 weeks. We are
continuing to try out a variety of equipment and assistive technology devices so that we
may find the best devices that can support this childs learning. The family is starting to
identify equipment they may want to purchasing using the Deaf/Blind Grant funds. This

DESCRIPTION: After a discussion with the family about signs of Autism, the family
indicated that the daycare reports that their child participates and engages with peers. In
order to address the discrepancies of the observation and to identify what each person
needs to do to support this child to participate and engage with others, I scheduled a
meeting with the teachers and parents. We addressed 4 goals and identified ways to
modify the curriculum and set up supports so the child can participate better. This
supports ISLLC Standard 2 which support school culture for learning and Standard 4
because I am collaborating with the community partners.

DESCRIPTION: The MHS Robotics team approached our department to ask if we had a
family with a child with motor needs who may benefit from an adaptive childs motorized
play vehicle. I coordinated the meeting for the high school students and my family to
meet. At this meeting, the Robotics team met the child and reviewed what kind of
adaptive needs the child would need to be able to make the motorize vehicle work for the
child. The supports ISLLC Standard 2 because it promotes the use of the most effective
and appropriate technologies to support teaching and learning and nurtures collaboration
for high expectations for student learning. This also meets ISLCC Standard 3 because it
helped to obtain technological resources for the student.

provide ITS services for. I met with the teachers to review June lesson plans and identify

create and implement plans to achieve transition goals, Standard 4 to help build positive
relationships with the families between teams, and Standard 5 to ensure a system of
accountability for the students academic success.

DESCRIPTION: I met with the Manhattan Mercury, MHS Robotics Team and the
family that I work with at the family home for the newspaper interview. The Manhattan
Mercury wrote a story about the the high school robotics team project of adapting a
childs motor vehicle for the family. This supports ISLLC Standard 4 because it
promotes the use of the communitys resources and builds and sustains productive
relationships with the community partners. It also supports ISLLC Standard 6 because
the newspaper article helps to influence political, social, economic and legal and cultural
contexts.

transition was not a typical transition as the parent and child had needs that needed to be
met in a different way. We held it at the Job Corp daycare and the the Part B staff came

Standard 5 because it ensures a system of accountability for every students academic and
social success.

promotes instructional capacity building and creating a personalized learning instruction.

information about development and the next steps the child should encounter in the next

caregivers.

DESCRIPTION: I evaluated hearing for a child on multiple occasions with a result of
refer for each hearing screening. I followed the guidelines set forth by the state and I
wrote a letter to the doctor to request a referral to the ENT. This supports ISLLC
Standard 4 because I am collaborating and building a relationship with community
partners, ISLLC Standard 5 because it ensure accountability for the students success by
following the state guidelines to write the letter to the doctor to request an ENT referral.
I am also demonstrating ISLLC Standard 6 as I am advocating for children, families and
caregivers by sending the letter to the doctor.
