AFFIDAVIT OF CONSENT

I, Vicente G. Batac, of legal age, married to Mary Grace G. Batac, and presently residing
at #2 Liko cor. Mariveles St., Highway Hills, Mandaluyong City, after been duly sworn in
accordance with the law, hereby depose and state the ff:

1. That I am the Father of Kara Mia G. Batac, age 16, who is applying for a Student Drivers
Permit at the Land Transportation Office;

2. That I am giving my daughter, Kara Mia G. Batac full consent and authority to apply for
such Student Drivers Permit;

3. That I am executing this affidavit to attest the truth of the foregoing facts and my
undertaking to give consent to my daughters application for a Student Drivers Permit.
