AFFIDAVIT
I, KATE KATRE, Filino, of legal age, single and a resident of Tamblot
Street, Tagbilaran City, Bohol, Philippines after having been duly sworn to in
accordance with law do hereby depose and say:

That on July 4, 2017 at about 9:15 in the morning, I was on my way to

Miag-ao Residence located at Tamblot Street, Tagbilaran City to collect Divino
Miag-aos payment for the Tupperware products he ordered from me;

That by the time I arrived at Miag-ao Residence at around 9:30 in the

morning, I knocked at their door and called Divinos name;

That upon knocking, I heard the loud and painful cry of Divino Miag-ao
asking for help;

That the door suddenly opened and I saw Ellena came out running away
from their house with blood on her pajamas. Her right hand was holding the
butchers knife and her left hand was holding a hotdog-shaped object;

That I rushed inside the house and looked for Divino;

That upon seeing Divino, blood was oozing out from his groin area and he
said Tabangi koKate kay giputol ni Ellena ang akong lagay! Sakit kayo, dalha ko
sa hospital! (Help me Kate! My penis shaft was cut-off by Ellena! It really hurts,
bring me to the hospital!);

That I called for an ambulance so that Divino will be brought to the

hospital;

That I am executing this affidavit of my own free will and volition that
nobody forcing me to do so;

IN WITNESS WHEREOF, I hereunto affix my signature on 5th day of July

2017 at Tagbilaran City, Bohol, Philippines.
