AFFIDAVIT
I, WINONA MARIE BAQUIAL, M.D., L.L.B, Filino, of legal age, single
and a resident of Dampas District, Tagbilaran City, Bohol, Philippines after having
been duly sworn to in accordance with law do hereby depose and say:

That on July 4, 2017 at about 11:00 in the morning, Divino Miag-ao was
rushed to the emergency room of Holy Name University Medical Center Inc.;

That I am the senior medical examiner who examined Divino Miag-ao;

That after conducting medical examination of Divino Miag-ao my finding

was the following:

1. Patient was awake, conscious, coherent and responsive,

2. In respiratory distress
3. Skin cold to touch
4. Clear breath sounds
5. Tachycardic
6. With soft non-tender abdomen
7. Shaft of the penis was mutilated with lacerations and active bleeding
8. No scrotal swelling
9. Full range of motion
10. Capillary refill time is more than two (2) seconds and strong peripheral
pulse;

That I prepared a medical report showing such finding;

That I am executing this affidavit of my own free will and volition that
nobody forcing me to do so;
