    Dear Social Security Officer,

    This letter has been issued to Exchange Visitors of our Department of State designated program for
    cultural exchange.

    This program permits J-1 Exchange Visitors to learn about the United States first-hand as well as to
    share their culture with Americans under the sponsorship of Intrax.

    At the same time, the Exchange Visitors are permitted to be paid by their US host companies. To
    support the request for a Social Security number, Intrax has instructed the Exchange Visitor to provide
    you with a:

    1. Valid passport;
    2. I-94 Arrival/Departure Card printed out from www.cbp.gov/i94.
    3. DS-2019 Form - Certificate of Eligibility for Exchange Visitor (J-1) Status.
    a. Signed in blue ink by an Intrax representative.

    We hope that the above documents will allow for a successful Social Security application.
