AFFIDAVIT OF LOSS

I, ALYZZA KARA P. ACABAL, of legal age, Filipino, married and a resident

of Villa Corito Subdivision, Surigao City, after having been duly sworn to in
accordance with law, depose and state:

That I am a graduate of Bachelor of Business Administration Major in

Management in Silliman University, Dumaguete City in 2013;

That said Diploma was missing and must have been lost sometime in April
2017 together with my other personal documents while travelling from
Surigao City to Davao City;

That despite diligent search and efforts to locate the said diploma, I could
not find the same such that I believe that it is now lost beyond recovery;

That am executing this affidavit to attest to the foregoing facts and to

support the application for the issuance of a new diploma in lieu of the one
which was lost.

IN WITNESS WHEREOF, I have hereunto set my hand this 5th day of
