To whom it may concern,

Please excuse our daugher, Camille Dueñas, for missing her classes on Friday, May 22,
2015. Camille has suffered from asthma for several years, and on Thursday night she
had an acute breathing episode that required us to take her to the Emergency Room.
Although her asthma attack was treated successfully, she needed to be monitored for
several hours and was therefore unable to attend school the following morning.

Camille’s doctor has given her permission to resume school next Monday, provided that
her breathing remains normal over the weekend and that she carries a rescue inhaler
with her at all times. We’ve also attached a note from the doctor for your convenience.

If it is necessary, Camille will be happy to catch up on any missed projects or


assignments once she returns to school next week. We appreciate your consideration
and would be happy to provide any additional information that you may require.
