This Waiver & Release of Liabilityis administered on 01- -2018 by the following

Japlit, Mike Shem Mortel, and April Galarosa from National University in favor
of ___________________________________. The activities that will be conducted are in
partial fulfillment of the requirement for CE Elective 2 – Earthquake Engineering.
The activities are as follows: (a) to acquire geographic set-up and information,(b) to
investigate earthquake evacuation protocol, (c) to examine the preparedness against
liquefaction and movement of the fault line.
We agree to observe and obey all rules and warnings and recognize the presence of
inherent risks associated with these activities.
We agree to compensate for the damages incurred caused by our irresponsible
actions.
We agree that we are under no pressure signing this agreement and have been
accustomed a reasonable agreement to review it before the formal signatory, and
have been acknowledged with the free will to have our authorized guardians review
this as desired so.
In such events that may cause controversies, we agree to resolve any dispute
arising through amicable negotiations.
