Dear Ma’am Daga,

Good Day!

I wrote this letter to explain why I had a non-validated school ID for the
second semester of the school year 2017-2018. In all honesty, I didn’t know that my
ID was not yet validated and I just recently found out about this matter when I went
to have my clearance signed by your good office. I even checked the list of students
who validated their school ID for the second semester because I thought I did the
same and followed the procedure when I enrolled for the second semester. I found
out that it was not the case.

As much as it surprised me, I know that I am at fault and I sincerely apologize

for not going to your good office for ID validation. It must have slipped in my mind to
do so since I was out of the campus most of the time because of our community
placement in Dagami, Leyte. Again, I humbly and sincerely apologize for failing to
comply with one of the directives when enrolling in the university.
