On the past school year, the undersigned was then once an employee/ faculty member of the department
you are supervising right now. Every requirement needed as per instructed by Department of Education
and by the school itself had been carefully polished and completely submitted already in the office but it
went in my awareness that some of the students under my instruction don’t have their grades since it is
not reflected on the grading system the school uses.

The undersigned would like to express that the turn-out of any document lapses or discrepancies are
beyond my control since it is really an assurance that I have encoded before I left the institution especially
every detail after all completion processes of the students. However, what is far more important right now
is the benefit of the students.

Hence, I would like to ask your consideration to reencode the grade of JADE D. VISBE, a grade 12
student of your department. To wit, his final grade is 80.00% incurred from his midterm grade
(75.00%) and final quarter (85.00%) in relation to his completion in my subject with the descriptive
title “Pagsulat sa Filipino sa Piling Larang” instructed in the 2 nd semester, academic year 2018-2019.
