LOCKOUT TAGOUT STANDARDS

LOCKOUT- physically neutralize all energies in a piece of equipment before beginning any maintenance or repair work
Locking switches and valves
TAGOUT- placing a visual warning tag or sign on an energy isolating device
(E.g.) Warning not to operate the machinery until the tag out device is removed.
Energy that could cause harm to any human being such as:
Electrical- (220 - 480 V, High Voltage – Industrial, Lighting).
Chemical- (Reaction, Fire, Explosion).
Kinetic - (Release of a spring, Moving parts, Falling Objects).
Thermal Radiation- (Hot/Cold, Radiation).
Pneumatics
Hydraulics
Pressure - (Pressurized Gases, Vacuum Pumps, Pneumatic Cylinders).
Other.

Necessity of LOTO:

To avoid unexpected energization of the machines


To avoid Sudden Startup of Machines/ Equipment.
To avoid Uncontrolled release of energy

JOBS WHICH REQUIRE LOCKOUT / TAGOUT:


Cleaning and oiling machinery
Rebuilding equipment
Machine “set up”
When servicing, maintaining or repairing mechanical equipment where parts could create a safety hazard.
When working on pipelines that carry hazardous chemicals (e.g. high pressure, toxic, corrosive, etc.).
On electrical circuits and systems where a high risk of electrical shock exists.

LOCKOUT-BOXES:

KEY CONTROL BOX PADLOCK CABINET KEY CABINET LOCKOUT CABINET

Affected Employee:
Is any individual who gets directly involved in a GEW, at any point of time during the course of the work.
E.g. rigger, fitter, welder, technician, operator, helper, contract labor and so on…
Authorized Employee:
is one who is officially authorized to apply Equipment Lock (EL) (Yellow Color) on an equipment/ facility under his department.
E.g. Electrician – PCC, MCC, Starter, DB
PTW Issuer:
A trained person who is the designated in-charge of the issuing department in the concerned shift and has been authorized in
writing to issue PTW in a defined block/area
