Dear Dr. Karim

Welcome to the family of APOLLO HOSPITALS – APOLLO HOSPITALS

In reference to your interview of 06 April 2006 we are pleased to appoint you as

Resident Medical Officer (RMO) at Apollo Hospitals Dhaka (AHD) on the
basis of following terms and conditions:

1. You will have to join Apollo Hospitals Dhaka by the April 2006.

2. You will be paid a Gross Monthly Salary of Tk. 25,000.00 (Taka Twenty
Five Thousand only) by the Hospital for the services rendered by you.
Following is the break-up of your salary:

Basic Salary 15,000.00

House Rent Allowance 7,500.00
Conveyance Allowance 1,250.00
Medical Allowance 1,250.00
Total Monthly Gross Salary 25,000.00

3. You will be on probation for a period of 3 (three) months.

4. Your salary will be subject to deduction of income tax at source (if

applicable) in accordance to the provisions of Income Tax Ordinance.

5. You will be entitled for 10 days casual leave, 14 days sick leave and 18
days annual leave.

6. In order to promote the welfare of the patients who seek your services at
Apollo Hospitals Dhaka, you will provide your services exclusively at the
Apollo Hospitals Dhaka and will not provide the said services in any other
place.

7. The code of conduct and ethics for doctors will be applicable to you and
you are expected to abide by the same.
 8. You will be required to be registered with the Bangladesh Medical and
Dental Council.

9. Apart form clinical / diagnostic professional responsibilities, you will also

take active participation in medical education, research and CME
programmes.

10.The Departmental Coordinator will coordinate with you in carrying out

his/her administrative functions and you will extend your full corporation
to him/her in this regard.

11.You will be required to enter into an agreement for ensuring at least one
year consecutive services with Apollo Hospitals Dhaka for the professional
development training program to be provided by Apollo Hospitals Dhaka. If
you are found in breach of contract you shall have to pay the amount
determined for compensation as per the agreement.
12.If you desire to terminate your services with Apollo Hospitals Dhaka, one
months notice or pay in lieu of notice shall be given by you to Apollo
Hospitals Dhaka.

13.You will maintain total confidentiality of all process and know-how,

security arrangements, administrative and / or organizational matter of a
confidential / secret nature, all company related information / documents /
files etc., to which you may have access directly or indirectly.

14.If and when information furnished by you in your application regarding

your experience, qualification etc., are found to be inadequate or not true,
you will be liable for termination or such other action as the Management
deems fit..

15.In all matters including those not specifically covered in this letter, you
will be governed by the service rules in force that are relevant to
personnel of your category. You will also be subject to rules and
regulations that may be framed from time to time.

16.Any change of your residential address should be intimated to us in

writing within one month of such change.
