Oxford United Healthcare.

Coverage at a glance:
Doctor’s visits: Primary Care: $ 40.00 ( ded waived)
Specialist: $70.00 (ded waived)
Individual/Family deductible $2,500.00/$5,000.00 applies to major medical.
Lab $ 20.00 no deductible.
Dental Insurance: Comprehensive dental insurance plan provided by Guardian Dental
Insurance.
EMPLOYEE CONTRIBUTION, deduction every pay period:
SINGLE $ 29.30
PARENT & CHILD $ 65.27
HUSBAND & WIFE $ 62.87

Commuter benefit – employee may contribute to pre-tax money for commuting related
to parking expenses and mass transit fares as per the limits set by IRS.

Supplemental benefits through American Family Life Assurance Company of Columbus


(Aflac). This includes personal cancer protection plan and personal disability income
protection plans and Employee Life Insurance, AD&D.

Time off package including 7 holidays, 10 vacation, 5 sick days and 2 personal days.
Additional vacation days are paid to full time employees based on their years of service
with the company.
