    May I apply to march for graduation as scheduled on March 22, 2009. I will abide by the conditions as agreed
    during the meeting for the graduating students held last March 2, 2009. The following are the conditions:
    1. I must be prompt in attending all scheduled practices starting March __________________
    2. My attendance will be checked at the start and end of the practice. If I come fifteen (15) minutes after the
    start of the practice, I shall be considered late but is allowed to join the practice.
    3. Three (3) tardiness is considered one (1) day absence.
    4. If I come twenty (20) minutes after the start of the practice, I am considered absent but is allowed to join the
    practice.
    5. I am only allowed three (3) absences to be able to march on the graduation day.
