JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, RIZA C. RODRIQUEZ and MARIA RENELYN M. FLORES, Both of

legal age, Filipino, married and residents of Brgy. San Roque, Macrohon,
Southern Leyte, Philippines, after having been duly sworn to in accordance
with law, do hereby depose and state;

1. That we personally know the personal identity of BERNARDITTE A.

MACALOS of San Roque, Macrohon, Southern Leyte;

2. That she was born on October 26, 1980 at Nasipit, Agusan del Norte;

3. That however, in her DSWD records, particularly in 4P’s ID Card, her

name was erroneously recorded as BERNADITTE A. MACALOS,
instead of BERNARDITTE A. MACALOS, the one being reflected in
her Certificate of Marriage issued by the Local Civil Registrar of Padre
Burgos, Southern Leyte

4. That the names BERNADITTE A. MACALOS per DSWD records and

BERNARDITTE A. MACALOS per Certificate of Marriage, refer only to
one and the same person;

5. That we know these facts because we live in the same barangay and
close friends of their family;

6. That we are not related to them within the fourth civil degree, either by
affinity or consanguinity;

7. That we are executing this affidavit to attest to the veracity of the

foregoing facts and for any legal purpose this may serve thereby.
