AFFIDAVIT OF CONSENT

I, MARIA LORNA PALARAS MALABARBAS, of legal age, married, Filipino and a

resident of Brgy. Jubas, Libagon, Southern Leyte after having been duly sworn to an oath
in accordance with law, hereby depose and say;

1. That I am the biological mother of ANGELO P. MALABARBAS, a two year old boy,
being born on February 5, 2016;

2. That I hereby give my consent to MARIVIC L. ELVIÑA, Filipino, married to Elmer

Elviña, resident of Brgy. Bugasong, Libagon, Southern Leyte to be an adoptive
parents of my son, Angelo Malabarbas;

3. That I consented this adoption for the benefit of the said child, considering that I
have five children to feed;

4. That I trusted the named spouses to be a good parents to my son;

5. That if ever, an adoption decree be granted I will renounce my parental authority

over the said child;

6. That I executed this affidavit in order to attest to the truth and veracity of the
foregoing and for whatever legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 5th day of May 2018 in the
Municipality of Sogod, Southern Leyte, Philippines.
