Category of Incident: Medical Treatment Case (Electric Burn)


Photo: Brief Description:
1. Electrical team were working for joint replacement of LT power cable
from Sugar Plant to Co-gen Plant.

2. On 3.35 pm power supply isolated from both ends. Permit No. 3788
and tag No. 79 applied at both ends with danger boards displayed at
sugar side panel as no provision of Lock Out is available at Sugar
electric panel.

3. Mr. Prashant Lole (Elect. Safety officer) issued a permit to work and
activity started. Mr. Chidanand Chougule (Electrician) & Mr. Sagar
Kamble (Eletrical helper) were cutting the cable. R Y B phase cable was
cut off but during cutting the neutral cable suddenly electrical flash over
occurred.

4. Mr. Chidanand Chougule (Electrician) and Mr. Sagar Kamble


(Eletrical helper) were got Superficial burn injury on hands.

Immediate Causes:
1. Casual approach as no auxiliary power supply (DG) of sugar was
ensured.
2. Written communication with client was not made.
3. Permission / signature of clinet was not obtained being a common
supply of Sugar and Co-gen.
4. No provision is available for LOTO applicability and work started
without Lock Out.
5. Electrical SLD of Sugar plant not studied before work.
6. PPE did not used.

Root Causes:
1. LOTO Procedure (OCP 65) was not followed.
2. Casual approach by electrical team and bypass the safety norms.
3. Reverse power in neutral of sugar MCC (Neutral floating) due to
common neutral was not ensured.

Recommenations & Lesson Learnt:


1. Provision of Fuse need to be made all poles and neutral.
2. LOTO provision to be made on panel and safety norms to be followed.
3. Unauthorized person entry need to be prohibited.
4. More knowledge about Sugar electrical SLD to be adapoted by Electrical team.
