Declaration

I wish to take all my examinations at the above institution. I will ensure the institution
contacts the Exams Office two weeks before the exams at the latest to confirm they are willing
to make the arrangements. I will take responsibility for any local charges by the overseas
institution. I will pay the overseas examination charge of RM300.00, and will ensure it will be
received by the Finance Office before the exams. I know that this payment is separate from
and in addition to any reassessment fees I have been informed about, and must be paid even
if my examinations are to be as a first attempt. I will also ensure that this form is submitted
to the Examinations Office one month before the exams. I understand that if these
conditions are not fulfilled my application to take my examinations overseas will
not be granted. I confirm that I am aware of the University's Policy on Overseas
Examinations.
