AFFIDAVIT
I, Xxxxxxxxxxxxxxxxxxxxxxxxxx(Name), of legal age, married,
Filipino citizen and a resident of ------------------------(Location), after
having been sworn to in accordance with law, depose and state that:

1. My name was reflected as the registered owner of

xxxxxxxxxxxxxxxxxxxxxxx in a document denominated as
Certificate of Business Name Registration filed before the
Department of Trade and Industry (DTI) for the year 2018.
Attached as Annex “A” is the said certification;

2. However, I already transferred the said business to my daughter

in law xxxxxxxxxxxxxxxx. As such I am declaring that she is
now the lawful and registered owner of the said business. As a
matter of fact, xxxxxxxxxxxxxxxx has been the lawful and
registered owner of the water station in the year 2010 to
2015.Attached as Annex “B” is the said certification;

3. I avow under penalty of law that the name

xxxxxxxxxxxxxxxxxxxx as appearing in the Certificate of
Business Name Registration for the year 2018 must be
cancelled and be rectified as xxxxxxxxxxxxxxxxx, the true and
registered owner;

4. I am executing this affidavit to attest to the truthfulness of the

foregoing statements and for whatever legal purpose it may
serve.
