I, the undersigned name is sending you this letter as notification that I am

allowing /authorizing the bearer of this letter, Mr. Joseph B. Cawaling my brother
to claim my Authentication copy of my board rating result in your office.
I won’t be able to claim it personally becaumportant reason that is why I have
asked her to bring this letter together with my identification card as valid proof. I
am also giving her the right to sign any papers that has connection with the claiming
of my Authentication copy.
