To whom it may concern,

Good day, I am writing to formally authorize Mr. James Vincent I. Dancel to represent
me and obtain the ticket for my oath taking ceremony on July 9, 2023. Unfortunately, due to
unforeseen circumstances, I am unable to be physically present at the Professional Regulation
Commission office to get the ticket personally.

Please be informed that Mr. Dancel has my full trust and confidence to carry out this task
responsibly and in accordance with any instructions or guidelines provided by your office. He
will be able to make any necessary decisions or sign any documents on my behalf in relation to
the ticket collection.
I kindly request your cooperation and assistance in facilitating the smooth process of ticket
retrieval for my oath taking ceremony. I understand that I will be held responsible for any
instructions or decisions made by Mr. Dancel during this matter.
