Authorization Letter for Yearbook Pickup

To Whom It May Concern,

I, Andrei Carl Serrano Marasigan a graduate of Adamson University with the degree of Bachelor
of Arts in Communication and student number 201111028, am unable to personally collect my
yearbook due to prior commitments.

I hereby authorize Ms. Jean Belarmino, a fellow alumna of the University, to collect my yearbook
on my behalf. Ms. Belarmino is aware of this arrangement and has my full consent to receive
the yearbook issued in my name.

Enclosed with this letter, please find a copy of my identification for verification purposes. I trust
that Ms. Belarmino will comply with all necessary procedures and provide any additional
documentation required by the university.

I appreciate your understanding and cooperation in this matter. If there are any further
