Greetings!

After having a meeting with the Grade 7 advisers and subject teachers regarding the 4 th grading
grades and final grades of the students for the recognition day, it was found out;

1. Mr. Norberto Española-

 the grade 7 Daisy adviser, does not have the grades in ArPan and EsP for the 4 th
grading period for the following section :7 Daisy( his own advisory class), 7
Jasmine, 7 Everlasting
 that the above teacher does not have a consolidated report of grades
 that because of this acts, fellow grade 7 cannot make their report too.

2. Mrs. Virginia R. Gumidam

 No MAPEH grades submitted to the advisers under her class; 7 Jasmine,7

Carnation, 7 Sunflower and 7 Everlasting both for 4th Quarter and Final Grade.

3. Mrs. Merly B. Cabaluna

 No Filipino grades submitted to the advisers under her class; 7 Jasmine,7

Carnation, 7 Sunflower ( her own advisory class), 7 Daisy and 7 Everlasting both
for 4th Quarter and Final Grade.

4. Mrs. Jaidam Jatulla

 She was absent today, however, the adviser of Grade 7 Dahlia informs the
undersigned that he have not yet received the Science grade sheet.

They were given the opportunity to comply everything today not later than 12noon but failed to
do so as of 3:00 PM, April 4, 2017.
