} Indonesian population 258 million (2016)
} 17.500 islands
} 5200 km from east to west
} 2000 km from north to south
} Air ambulance movement is time sensitive
and require experienced operator to ensure
the highest level of safety for the patient.

5
} Time critical transfers are faster when
serviced with air ambulance like helicopter
compare to land ambulance
} But air ambulance transfer require additional
steps of aviation factors such as flight
planning ( weather and traffic, aircraft roll out
and start up and ATC limitations) .

6
} High ambient noise levels
} Limited working space
} Not possible to perform some
procedures like auscultation
} Limited visibility
} Sunrise to sunset

7
} Dont have pressurized cabins
} Transport just one – two
patient
} Have stricter weather
minimums
} Commonly dont fly >10000
feet ASL

FLY STRESS FACTOR
} Altitude
} Hypoxia
} Vertigo

} Vibration
} Stress

9
◦ Cervical spine stabilisation - special braces need
to be used as a sandbags .
◦ A prophylactic intercostal catheter may need to
be considered if there is any pneumothorax or
more than two ribs are fractured as a
pneumothorax may expand at altitude.
◦ Haemorrhage should be controlled.
◦ Fractures should be splinted (preferably without
air splints).

10
} Full arrest
} Terminal stage
} Active infectious disease
} The patient dont want the transfer
} Unstable patient
} Stable patient when land vehicle is more
appropriate to do the transport

11
} Challenges
} Affordability
} Infrastructure, Hospital Helipad
} So much restriction from regulation side
◦ NO Night VFR
◦ Sunrise to sunset operation only
} High operating cost ($ 10,000/hour mission)
