Hse Alert: Excavator Incident

Excavator Incident No: HB/32/2016


BRIEF INCIDENT DESCRIPTION:
After cleaning excavator cabin, IP was coming out from his cabin for tea break. To climb down from
cabin, he took support by holding the door with his right hand & jumped from the cabin. As his right
hand finger got caught in between door and door frame, it resulted into cut injury. IP was given first
aid at site and further taken to nearest hospital for further medical treatment.

WHAT CAN YOU DO?
 Outside support Handle was  All excavator door should have glass as well as handle to hold
missing. it from outside.
 Standard inspection  Operators to be informed through TBT that while climbing
checklist does not emphasis down from excavator they should position the cabin slightly
specifically on handle & glass tilted from crawler in order to have sufficient space to climb
requirements down from cabin.
 Equipment inspection checklist should also cover handle
(inside & outside) and glass door arrangement.
LEARNING FROM THE INCIDENT:
 Operator cabin should be fully covered with glass door and handle as
per manufacturer guidelines
 Position of cabin and crawler should not be in same line in order to
provide proper foot hold while de-boarding.
