 What is a Permit to Work?
 A Work Permit is a formal written document used to control certain
types of work (which may have potential of accidents).
 It is also a means of communication between Management, Plant
Supervisors, Operators and those who carry out the work.

 Advantages of using Work Permits
Ensures appropriate people are authorized to carry out
designated work.
Makes it clear to people carrying out the work the exact
identity, nature and extent of the job and the hazards
involved.
Specifies the precautions which need to be taken.
Ensures that the person in charge of the area is aware of all
the work being done under the work permit system.
Provides a formal handover procedure for use when the
permit is issued for a period longer than one shift.

 VALIDATION PERIOD

PTW validation period has been revised; revamped

procedure has following validation period
 Maximum time for validation is 03 normal Operations shift. (Normal: 8

hrs shift 02 extensions of 8 hrs & 01 extensions for 12 hrs shifts)

 Overall timing shouldn’t exceed 24hours in any case.

 Revalidation to be done at work place & within 2 hrs of next shift

start by USE, Operator & technician at work place.

 Permits should be closed within 02 hrs. after completion of Job.

 Applicable on all permits; irrespective of their types.

 Permit to Work Authority Matrix

PTW can only be issued and accepted by all authorized individuals.

There are certain permits which requires elevated level for issuer /
acceptor or authorization considering the associated hazards / risk for
specific job. Below matrix is developed ensuring appropriate skill;
experience and authorization level of issuance and acceptance of permits
to be ensured and complied for safer handover and execution of job.

Note:- In Major outages/Turnarounds when area is declassified, all authorized

acceptors can accept all type of PTWs including mentioned above except clause “7.b, 7.d;

ignition, on prescribed plant or equipment.

A Hot Work Permit is required for any job where the heat
generated is of sufficient magnitude and intensity to cause
ignition of any flammable liquids, gases or any other
material.

• Preparations of HWP will be under CWP.

• If Hot work related jobs required JSA will be accepted /

issued not lower than Sub Engr level.

 Hot Work Permit Exemption
During turnaround / Shutdown, if there is no fire or explosion
hazard in area then on the recommendation of UEIC, the
Production Manager of the relevant site can declare area as
declassified and may obtain HSE waiver for following;
• Photography can be done under cold PTW.
•No PTW required for Photography with classified camera in
operating areas or in control rooms building however, prior verbal
approval has to be taken from USE /SEIC.

 CONFINED SPACE ENTRY WORK
PERMIT
A Confined Space Entry permit is to be obtained before any person enters a location defined
as Confined Space Entry.
Validity:- 24 Hr max.
Authorization:- USE can issue CSEP followed by approval by the SEIC. Only relevant
section Engr / Sub Engr will be acceptor for other then open pit, skirts etc.
Confined Space Entry Standard
Examples of confined spaces include tanks, vessels, certain pits, ditches, sewers/septic tanks,
excavations; exhaust stacks and vessel support skirts and all excavations greater than 3.5
feet in depth. Entry will be allowed only after compliance with the specific guidelines of
this standard.
 It is mandatory to provide proper and safe means of entry and egress for all confined
spaces. This Rule is applicable for all confined spaces, whether these are permit required
or non-permit required.
 Special Considerations, Prior to confined space entry, all interfaces involved in
confined space entry must discuss / agree on the safety precautions to be taken for
safe execution of the job.

 CONFINED SPACE ENTRY
PERMIT
Isolating Confined Spaces:- Physically disconnect or blind all process material lines,
including nitrogen purging lines. Put blind as close to the vessel as possible.
For shared stack / duct; the duct or stack to be disconnected or blinded.
Lock-out
All power driven equipment as associated with the permit must be LOTOTO
Agitated / rotating equipment must be disconnected & tagged from power source.
General Requirements;
 A safety harness and rope for every individual who will enter the confined space.
 Authorized individuals are allowed to enter only. (list will be available with Standby
man)
Secure Rotating Equipment
 The drive coupling must be disconnected or.
 The leads at the motor must be disconnected.
 Shafts for fan blades or agitators must be secured to prevent their movement before
anyone enters the tank if a draft or other force can produce free wheeling.

 Electrical Work Permit (Purple Card)
Required for any work electrical system inside Sub Stations including grounding of
system.
Note:- This excludes visual inspection of motors; electrical isolation of Motors or
miscellaneous electrical field related work, which will require a cold Work Permit.
Validity : 24 Hrs Max ( 2 validation for 8 hr shifts or 1 revalidation of 12 hrs shift)
 Electrical team will take care of job themselves however job will be informed to O&U
operations & SEIC

 DCS , ESD Systems ,PLCs and Energized Field Instrumentation
 Work inside marshaling cabinet; Annunciator panels
 Other instruments & control systems e.g. Anemometer, Recorders, Vibration Monitors etc.
 Safety Critical devices; USE will initiate; Instt Engr / Sub-Engr shall fill and co-sign the E&I
related part. Approved / authorized by the SEIC keeping UM in loop.
 Non-Safety Critical devices : USE can authorize; while Inst Engr / Sub Engr or Authorized tech to
cosign the Work Permit after filling the E&I related part.
 Work on energized devices; less than & equal 24V, USE to initiate & accepted by respective
instrument acceptor.
 energized circuit 24 ~ 220V; initiated by USE & authorized by SEIC and co-signed by area
Instrument Engr / Sub Engineer / Supervisor.
Note:
a) A cold work permit shall be used for work on field instruments (Local PG; TG; Cable laying
etc.). All jobs associated with ESD, DCS or even field electronic instrumentation should be
covered under Instrument permit.
b) Permit for changing the set point or the logic of Safety Critical devices will be issued only if
the approved “Safety Critical Device Set Point/ Logic Change Form” is available.
c) Note: - Work on TE (thermocouple) will be considered de-energized.

 Instrument Work Permit General
guidelines
 General guidelines; Following guidelines should be followed while carrying out
an instrument related job;
 Electrical lock out, tag out or own isolation must be fulfilled before issuance of PTW.
 PM on any single security loop will be covered through one work permit. However,
safety critical defeat precaution is to be taken as per procedure.
 For any hot work; separate Hot PTW to be obtained addition to Instrument permit.
 A separate electrical permit must be issued for electrical disconnection of circuit
breaker leads where required. (separate rack out procedure/ card)
 Ensure that the work area is free of any toxic gases and the area condition is
sufficiently open to allow for adequate ventilation.
 Boardman’s signature is MANDATORY for work on controllers / control valves.

 Vehicle Entry Permit
Vehicle entry permit to be obtained before any vehicle enter B/L of operating
plants or PTW areas.
Authorization:- USE will be issuer; accepted by Motor Pool or relevant section
authorized representatives.
Vehicle operator will sign the permit after thorough understanding of the job.
For routine chemical / Oil shifting without lifting as per Operations
requirement should also be covered under PTW.
Single Vehicle Entry Work Permit is sufficient for single vehicle for a single
area.

 Vehicle Entry Permit
Chemicals / Raw Material unloading tankers / trucks; are allowed to
travel on non PTW roads / non process areas provided allowed by respective
Area Owner / USE to enter in non PTW areas / roads.
 In case of entry towards PTW roads / process areas PTW will be required.
SEIC will issue the permit and to be accepted by respective area Owner
/USE
 Note: - a) If a vehicle such as fork lifter or pay loader is used for
transferring material inside the plant battery limit, an additional cold
work permit will not be required.
 Do not drive through a cloud of Ammonia or any other combustible
gas / vapor, even if a road barricade has not been put up.
 No Road barrication is allowed without prior formal information to
SEIC;,ERT and area owner and formal approval from SEIC after
ensuring this information dissipated to all concerned.

 Lifting Over Process Equipment Permit
This permit must be obtained before handling of any load over process equipments by
Crane or other lifting devices (fork lifter, hydra lift, pay loader etc.).
Authorization; Issued by USE ,if there is no potential of PSI. accepted by authorized
acceptor of Maintenance / Project or relevant section.
 In case, a potential for PSI is present permit will be filled by USE and co-signed by
Maint Engr / Sub-Engr (defining load path where boom swing) and then approved
by the SEIC.
 Incase of 4 MT weight or above it will be verified and co-signed by Maint Engr
along with precautions and rigging plan. For lesser weight, the permit will be issued
by USE
 Separate permits of Vehicle entry & LOPE will be issued.
 Permanently mounted hoists, overhead cranes, and lifts covered by an area
procedure are exempted from requiring this permit.
 Lifts made with chain blocks do not require this permit.

An additional Cold Work Permit will not be needed for the

lifting over job.

 Excavation and Civil Work Permit
(White)
 Excavation PTW must be obtained for all excavations greater than SIX inches in depth.
Initiated;- Within the process units’ battery limit by USE and Co-signed by Project
Incharge or designate, Electrical in-charge or his designate, IT In-charg
 Excavation by powered equip is prohibited closer than 4 feet to any underground
cable/pipe.
 Initial digging in proximity to bricks must be done cautiously by hand. An Electrical
Work Permit for excavation at or near electrical equipment is required in addition to the
Civil and Excavation Work Permit.
 Red bricks must not be broken or cut without written approval of the Electrical UM.
Validation:- revalidation after 15 day by all interfaces.
 Shoring:-
 All excavations greater than 3½ feet or closer to a wall than the depth of the excavation
shall either be shored or excavated in ½: 1 side slope to protect the employees from cave-
ins or sliding material.
 In locations where sub soil water is encountered at a depth less than 3½ feet, shoring shall
be done at all depths.

 Radiography Work Permit
This permit is intended prior to conducting any radiography in the
area to ensure protection for people from exposure working in the
surrounding area.
Authorization; Radiography Work Permits will be initiated by the
Inspection Engineer/Sub Engineer and approved by the SEIC after
it has been cosigned by the USE and accepted by Inspection
personnel.
 Normally, a 24 hrs.' notice regarding intended place & time of
radiography work will be given to Prod Dept. by inspection
except in emergency cases.
 The UM / SEIC should ascertain that area is cleared of people
prior to radio grapy via announcement through PA or Available
means.

 Radiography Work Permit
•an adequate number of warning signs shall be conspicuously
displayed on the out-skirt of the cordoned area
•A physical radiation survey around/outside of the cordoned off area
shall be carried out during radiographic operation. This is usually
accomplished by using a radiation intensity meter
•Prior to radiography operation, the radiographers and their
assistants shall ensure that no unauthorized personnel are within the
radioactive sphere
•At all times during the exposure, a competent and qualified
radiographer must be at the site. He must be capable of returning
the isotope to its camera at any time when circumstances so warrant
•On completion of radiography, Area operator will ensure that all
barrication tapes and signs are removed
The annual allowable exposure limit is 5.0 REMS

 Lock Out Tag Out
In revamped SOP; LOCK out; Tag out and try out is being
proposed as per OSHA guidelines. Following is brief update
• Breakers to be locked as per existing practice by all involved discipline.
• Filed isolation will be supplemented by lock & Tag on all valves by operations.
• Group Lock out to be followed (Keys of all locks to placed in a box followed by
lock by operations.)
• Key box will also be locked by all discipline involved in job on equipment.
• Upon job completion & PTW close out discipline will remove their lock.
• Operation will remove their lock in last to remove field isolations.
• Incase of interplant PTW; requesting plant’s lock key will also be put in group
locks box.

 PHYSICAL ISOLATION
Double Block & Bleed Isolation
Physical isolation procedure proves impractical in majority of areas due to
design constraints at all plants. Following guidelines will be followed:
 If the circuit downstream of I/V to be dismantled with only one I/V available
then after dismantling, pressure blind must be applied till reinstallation.
 If the circuit downstream of I/V not to dismantled, job can be handed over on
single isolation (W/O Blind ) after LOTO; provided NO passing of I/V.
 If blind installation is not possible; job can be handed over on single I/V with
LOTO provided there is NO passing of I/V and job verification by SEIC /
UM. Permit for such Jobs to be authorized by SEIC only.
 In case of single isolation with passing problem or in case of block & bleed
with both valves passing then in order to perform the job, JSA must be
