    GUIDELINES ON 1 YEAR MEMBERSHIP

    1. To avail of the 1 year membership lock in package, either of the following is required:
    a. Payment of the full 12-month amount.
    b. Complete PDC’s for the 12-month period.
    c. Initial payment of the first and last month.
    2. Member understands that when he/she avails of the 12-month membership package, membership cannot be
    cancelled, or transferred.
    3. Freezing/Holding of account details:
    a. Member MUST sign and complete the Freeze Form at least a week before it takes effect. Verbal
    confirmation will NOT be honoured. Forms are with the admin staff.
    b. An account can be put on hold for a minimum of 2 weeks and a maximum of 1.5 months (consumable)
    within the 12-month period.
    c. A member can freeze his/her account anytime with a minimum of 3 days per freeze.
    d. Sundays are counted on the number of days of the freeze period.
    e. Member is required to give a medical certificate of injuries and illnesses for documentation.
    4. Payments are NON-REFUNDABLE.
