II. SCOPE OF WORKS

1. PT. Bill Emas Sarno Provide as follow:
1.1 Required NDT equipment, Reporting, and Procedure (the specification is specified by client).
1.2 Required NDT personnel for NDT execution.
1.3 Required NDT consumable materials.
1.4 NDT Radiography Testing (RT), Ultrasonic Testing (UT), Magnetic Particle Testing (MT), and Dye Penetrant
Testing services for PLTU Batang at Central Java.

1.5 Personal Protective Equipment (PPE).

2. Client provide as follow:

2.1 Scaffolding, lighting, suitable power, water, lifting, cranes, stairway if required for NDE and/or access and
safe working conditions at all times
2.2 Dark Room and clean water line for darkroom if processing at site.
2.3 Report Format and Acceptance Criteria for Testing.
2.4 Office facilities including reproduction and information Communication Technology hardware and
software for business propose onsite
2.6 NDT Request and Drawing including flag/ sign material for inspection.
2.7 Accommodations and Mob Demobilization Technician (Mess, Meal, Local Transportation) to and at Site
2.8 Emergency and first aid facilities to include MEDIVAC to nearest suitable hospital/clinic facility onsite.
2.9 Written Contract /Agreement of prior to mobilization

III. TERM AND CONDITIONS

1. Above prices are excluding any applicable government taxes and withholding tax;
2. Rates do not include for any licenses, duties, retention, performance bond or other charges, which
would be to clients account
3. All rates are based on a 10 hours/day and any client approved overtime will be charged at the
straight rate prorated.
4. Minimum charge working technician 7 days/ trip
5. Down Payment 50% from minimum charge technician prior preparation and mobilization;
6. Payment shall be done within two (2) Week’s after receiving invoice, by bank transfer to our
company account;
7. This quotation is valid for thirty (30) days from the date of issued;
8. Delivery time, seven (7) day after received purchase order (PO); excluding preparation working VISA
9. Please issue the PO/WO before starting the job.
