Lingayen Pangasinan , and I JOY LAURETA , Filipino Citizen , of legal age , married and with residence
address at Pangapisan North Lingayen Pangasinan after having duly sworn to in accordance with law ,
hereby depose and state: THAT -

1. We personally know the live-in partners ANGEL ACE IRIS VINLUAN DE GUZMAN and SWEET
JENESIE VENTAYEN MIRANDA who begot a son named ZEUS ADAM ACE MIRANDA DE
GUZMAN who was born on May 2,2014
2. We are not related by consanguinity of affinity to the child's parent's;
