AGENDA:
1. Purchasing 1 set of Baseball Equipment with a cost of PhP16,000.00 (Wilson Brand)
 GPTA President, Nestor O. Bariquit, humbly asked the permission of getting
PhP11,000.00 from the School’s Development Fund since the remaining Sports
Fund is only PHP5,000.00.
 The getting of amount from the School’s Development Fund was agreed by
everyone. It was moved by Nino Ricablanca and duly seconded by Nestor B.
Onden.
2. Firming up the learners’ paying of Miscellaneous Fee
 The school principal, Mr. Norman E. Angay thought of giving an award of
recognition for those teachers who collected big amounts from the Miscellaneous
Fee, serving as a motivation for teachers in firming up the learners’ paying of the
said fee.
 The following teachers were announced as the awardees for having big
collections from the Miscellaneous fee in no particular order:
 Marilyn Badlon
 Charon Pateño
 Ella May Tomogsoc
 Resty Tanoy
 Jeevelou Calog
 Encarnacion Machete
 Lalaine Olavides
 Mary Ann Macion
 He also plans that the Homeroom President in each classroom will also be given
an award of recognition for the next School Year; 2019-2020.
3. Illnesses/diseases that will qualify for Medical Assistance
(PhP 3,000.00- Junior High; PhP 1,500.00- Elementary)
 The following are illnesses that will qualify for Medical Assistance
(Requirement: Hospitalized for 5 days and above)
o Applies for the requirement
 Does not apply for the requirement
o Dysentery
o Dengue
o Meningitis
 Rabies (if the owner of the dog is not identified)
 Accident
o Typhoid fever
o Kidney failure
o Other major operations
4. GPTA Treasurer Mrs. Jane D. Garcia’s Financial Report
5. Teachers will be given 10% from the collection of the School’s Development Fund for the
school year 2019-2020
 This is to entice the teachers to encourage the pupils and students to pay their
school obligation.
 This was moved by Mrs. Analiza Sanchez and duly seconded by Mr. Nestor
Onden.
 To encourage the pupils and students to pay their school obligation, it has been
agreed that the GPTA Treasurer will be present during the enrolment and in the
absence of the GPTA Treasurer, the secretary will take over as an assistant
treasurer. This was moved by Mrs.Gina Patria and duly seconded by Vengie
Paman.
 In connection to this, the GPTA Treasurer will issue a temporary receipt upon
payment.

6. Financial Report from the Teacher’s Classroom Project.

 Data were orally presented
 Only grade 9- Charity under the advisory of Mrs Mishell A. Yuson and grade 9- Justice
under the advisory of Roxanne Labrador failed to put up a project for the school year
2018-2019 under the following reasons:
 As for Grade 9-Charity, the previous adviser failed to give the money
collected for their ceiling fan project to Mrs. Mishell Yuson, the current
adviser. As the current adviser, she’s still trying to contact the previous
adviser to get the students’ contribution for the project.
 As for Grade 9-Justice, the previous adviser gave the money collected for
their project by March 2019 to the current adviser Mrs. Roxanne
Labrador. Since it was given almost at the end of the school year and the
students may not be able to make use of the project they have been
contributed, so the current adviser decided to give it all back their
contribution.

5. Brigada Eskwela on May 20-25, 2019

 The School Principal, Mr. Norman E. Angay humbly asked the GPTA officials to attend.
 Parents/Guardians who failed to attend, an amount of PHP100.00 will be charged.
 The payment of the said charge will be received by the GPTA treasurer.

Since no more agenda to be discussed, the meeting was adjourned at exactly 11:03 am.
