FOREWORD
Praise be to Allah for all His mercy and grace so that I can finish at the HSE
Job Training PT Bandar Abadi-Ship Builders and Dry Docks. Prayers and greetings is
always devoted to the Prophet Muhammad who has always been a role model for
humanity until the end of time.

Job done as academic requirements to complete the Job Training Courses in


the Department of Industrial Engineering, Faculty of Engineering, University of
Andalas. Practical work starting on December 18, 2017 until January 18, 2018. Work
Practice begins with the introduction of the company until the completion of activities
associated with problem reports contained in the company. Work Report is inseparable
from the support of various parties. Therefore, the author would like to express respect
and gratitude to :

1. Parents writers who provide support and prayers so that the author can execute
with good Job.
2. Jonrinaldi, Ph.D as Chairman of the Department of Industrial Engineering,

Details Event Description : Today is the first day of practical work in PT Bandar
Abadi-Ship Builders and Dry Docks, which started at
08.00 am. First we are instructed by Mr. Wenpri as
HRD to complete the data that has been given and a
number of documents as a condition for carrying out
practical work in PT Bandar Abadi. After giving the
document, we were placed in the HSE (Health Safety
Environmental) section. Firstly we are given guidance
on the rules that we must obey during being an intern at
PT Bandar Abadi. At 09.30 am induction is done to
provide some rules to enter PT Bandar Abadi area and
to know the danger and any work accidents that can
happen. After the induction, we got acquainted with the
staff and employees who are in the HSE section.
Our first activity after induction was the giving of
training by Mr. Arnold as the mentor of practical work.
The first training materials were given entitled "The
Role of Management in the Control of Occupational
Hazards and knowing 5 cycles of SMK3 (System of
Occupational Safety and Health) and 12 elements of
SMK3 audit"
SMK3 is part of the overall corporate management
system in the context of risk control related to work
activities that is useful for creating a safe, efficient and
productive workplace

Title Event / Activity : Field observations, making a report based on
observations and Training on Basic Fire Fighting
The second day of practical work starts at 08.00 pm.
Details Event Description : The second day of practical work begins at 8:00 am. The
activities carried out were observations on floating dock
and graving dock areas. In the area there are some tools
used to work in unsafe situations. For example, cable
conditions are found that are not safe to use and found
dangerous material in the floating dock area. After
making observations, we make the results of the
observations that have been made. The results of these
observations are attached to Appendix II
The next activity is the training given by Mr. Arnold
about "Basic Fire Fighting". This training provides us
with knowledge of the basic factors of fire occurrence.
In addition we are equipped with fire fighting
techniques in various fire classifications, so that the use
of fire extinguishing equipment and facilities can be
functioned as much as possible and able to reduce the
impact of fire risk that occurred.

Safety (BBS) and presentation on BBS
Details Event Description : Today our first activity was the introduction of
Scaffolding. The goal is that we recognize potential
hazards in the workplace at altitude. We also discuss the
use and maintenance of tools for fallen prevention.
Scaffolding itself is a temple building that is made
temporarily and used as a workforce buffer. We
understand the basic elements of scaffolding, the
dangers that can occur when using scaffolding that is
falling from the height and how prevention work
accidents occur.
Our second activity is the training given by Mr. Arnold
about "The Basic Occupational Safety Practices
(Behavior Based Safety). The concept of this training is
the dominant factor that causes work accident is the
human element. If the employee's behavior is unsafe
then it will result in an accident. If working in good
treatment, it will improve the quality of work and make
a profit for the company and workers' welfare. If the
behavior of workers is not good will likely decrease
productivity, quality will even cause harm to the
company and the workers themselves.
After understanding the concept of BBS, we made a
presentation about the BBS to the staff in the HSE
Office. In this presentation we get input in how to
deliver the presentation, such as intonation in the
voices, how to interact to the audience, and also how to
deliver the material so that the audience is interested in
the material we provide.

Details Event Description : On this day our activity is introduction about making
daily report and weekly report. Daily report is divided
into two parts namely leading indicators and lagging
indicators. On leading indicator there are HSES
meeting, safety induction, safety training, safety
inspection, TRA meeting, safety drill etc.
In lagging indicator there are fatality, lost time injury,
restricted work case, medical treatment case, first aid
case, near miss incident, near miss accident, etc.
When creating a weekly report, a daily report is required
to be cultivated. Likewise in making monthly reports,
which required weekly reports to be cultivated.

Personal Sea Survival Technique
Details Event Description : On this day, the activity we do is making monthly
report. In this report there are some indicators that are
general HSE, leading, environmental, zero rule, etc.
The next activity is the training given by Mr. Tigor as
HSE officer. This training explains how to survive at
sea. There are 7 steps to survival in the ocean:
recognition, inventory, shelter, signal, water, food and
play. The dangers of survival are foot cramps,
hyportemia, seasickness, brine burns, constipation, and
extreme cold skin damage. The survival priorities are
protection, feeding, and medical care. Personal life
saving appliances are using life jackets, immersion
suits, thermal protectors, visual signals and audio
signals.
At the end of the training, Mr. Tigor presents some
videos about personal sea survival techniques and
shows the practices that are done when we are in the
middle of the sea

Title Event / Activity : The introduction of Hot Work Permit, Observing
induction process, understanding of H2S hazard, and
cause of work accident.
Details Event Description : On this day, the activity we do is the introduction of hot
work. Hot work is work done on flames, welding,
testing work without explosion with electrical
equipment and other heat sources, which may cause
burning. we also know the personal protective
equipment used in hot work.
After that we see the induction process. Induction is the
introduction of the basics of occupational safety and
health to new employees or visitors made by HSE
employees. In this induction provide a review of the
rules that apply to PT Bandar Abadi as prohibited
smoking in the area that has been determined. And also
provide information about working procedures that
exist in the area of PT Bandar Abadi.
After that we were given an understanding of the
dangers of H2S. H2S gas is a non-colored, toxic gas that
can only be recognized by its odor at low
concentrations.
After that we were given an understanding of the cause
of the accident. Some of the causes of accidents are lack
of commitment to the management, poor control
engineering, unsafe actions and poor control.

Title Event / Activity : Presentation about Hot Work and Confined Space and
Observation on the Intan Daya 7 ship.
Details Event Description : On this day, the activities we do are presentations on hot
work and confined space. Confined space is a room
where it is not large enough and where it has such a
simple configuration that workers can enter to do the
work. Confined space has limited incoming or outgoing
access, limited air, limited lighting and more. When you
want to work in a confined space, list the required
equipment, check equipment and lighting conditions.
Personnel responsible for the work on confined space
are supervisors, entrants, attendants, and rescuers.
These personnel have different responsibilities and
tasks. Supervisors coordinate into the enclosed space.
Entrant where the workers are directly into the closed
room to carry out the work. Attendants are workers who
are outside the closed room and monitor the incoming
workers, and also keep the room against unauthorized
persons entering. Before entering the confined space,
checks are done. Useful to ensure that all workers
involved in a confined space have followed standard
procedures, and a work permit is required. This work
permit must be met before the work begins.
After understanding and presenting about hot work and
confined space, we were given a quiz. Where is this quiz
about hot work. After checking the results of the quiz
we did and get a good score. then we are allowed to
enter the ship. The ship we entered was Intan Daya
Daya 7 which was accompanied by Mr. Arnold. On
board, we saw ship parts like bow, chimney, stern,
spotlight, main deck and so on. At the time of
observation, we see workers are working hot work is
welding. Where the workers use personal protective
equipment such as helmed, apron, welding glove, face
shield and safety shoes. And we also see workers
working on confined space. There are attendants who
keep outside the room and there is a useful blower to
add oxygen levels in the confind space so that workers
who are not inadequate oxygen while working.

Title Event / Activity : Know how to calculate the oxygen content in the
confined space.
Details Event Description : On this day, our activity is to consult with Mr. Arnold
in preparing the report we will make. After
consultation, we discussed how to calculate the oxygen
content in the confined space.
Normal air calculations use breathing apparatus.
Normal air to do the work of confined space is 20.9%
or 21%. If a large air value of 21% then it is necessary
to supervise because it is not good if working with
excess oxygen levels. And if the air is small from 19%
it can cause shortness of breath and if its air content
close to 0% then it can cause death. Likewise with
oxygen levels of more than 23%.
Then we ask about the calculation of the working
duration in the confined space. Calculating the duration
of this work begins by determining the contents of the
tube by calculating the air capacity in the tube
multiplied by the pressure, which is usually the
maximum pressure on the tube of 300. After obtaining
the results, then the full duration is calculated by the
result of the contents of the tube with the average person
breathing per minutes, usually the average person
breathing is 40 minutes. After getting the results, the
number of durations is then reduced by the safety
margin which is usually worth 10 minutes, and obtained
the work on the confined space

Title Event / Activity : Learn the general permit
Details Event Description : On this day the activities we do look at the general
permit, which has been filled by the HSE inspector. On
permit there is the name of the ship, work location, start
work and finish work. There are several types of
permits inside such as permit painting, confined space,
blasting, miscellaneous work, and hot work.
At permit painting there are 4 stages: applicationby
foreman / supervision, endorsement by safety personel,
approval by project superintendent and notification of
completion of work by trade supervisor. On the permit
can be seen the type of hazard that can occur at the work
site. At hot work permit sketches or manual drawings
are attached to know where the work location is on the
ship. On the filling of work permit, attached the name
of the worker, the person in charge of safety and
supervision by attachment of the signature.

Title Event / Activity : Observation of possible hazards
Details Event Description : On this day, the activities we do are observations made
on board Intan Daya 7 by Mr Anton as Ass Head Officer
at 08:45 am. At the time of observation, we were told
the parts on board such as hold, maindeck, forcastle,
wellhouse, poobdeck, main mast, bull walk and so on.
As we rounded the ship, we found an apar with a low
condition marked in the direction of a red needle. but
the seal is still installed which means the apar has not
been used at all. That means apar leak. After that we
discussed about the type of fire that consists of class A,
class B, class C, and class D. In the class has different
functions and tasks. After we discussed fire
extinguisher, we descended the 7th diamond ship and
went to the alpha kirana ship. The alpha ship kirana is a
ship that is being repaired. On board, we see workers
working at altitudes using illegal scaffolding, as they
are not equipped with the standard of scaffolding. The
basis of scaffolding is ledger, standard, transom and
base plate. The workers do not use self-protective
devices properly as they do not use appron. As we
boarded the ship, we saw a confined space open. The
reason is that if the ship is on land then the confined
space must be opened so that the confined space is not
hot and lacks air. If the confined space is closed then it
can cause an explosion due to heat. After we surrounded
the ship Alfa Kirana, we went down the ship and went
to see the new ship building. After seeing the new
shipbuilding, we had a discussion about the accident
that happened at PT Bandar Abadi. There was an
accident that occurred due to the workers doing the
work but did not have permit. Doing painting and
welding jobs simultaneously. So there was an explosion
that left 5 workers dead.

workshop. Workshop is the creation of ship parts. In the
workshop there are several processes that occur such as

At the time in the workshop we are guided by Mr. Reno
as HSE staff who are on duty in the workshop area.
While in the workshop, we saw the shipbuilding
process. As well as the dangers that can occur when
work at the workshop is done.

Title Event / Activity : Safety induction, discussion of reports and introduction
of domino effect
Details Event Description : Today our activities are following the safety induction
given to 12 new employees. Safety induction is given
by Mr. Arnold who describes the procedures,
provisions and personal protective equipment
applicable in the company. The goal is for new
employees to better know the scope of the company and
its provisions.
The next activity is we discussed with Mr. Arnold
regarding the work practices report. There are some
editors that need to be fixed.
After that, we were given material about hierarchy of
control and domino effect. There are 5 hierarchies of
control: eliminate, subsitution, engineering control,
administration and PPE (Personal Protective
Equipment). After understanding the hierarchy of
control, we were given material on "incident and the
domino sequence". This material describes what is
incident, accident, near miss, recodable cases, lost time
accident, non lost time accident, and non loss time
accident.
Domino effect is divided into 5 namely Domino 1
which is the weakness of control or control in which
there are management control, pre-contract control,
contractual control, and post contract control.
Domino 2 is the basic cause of accidents or individual
factors and workers. Individual factors consist of
physical, mental, lack of knowledge and other
ssbagainya. The working factor consists of supervision,
engineering, tagging, maintenance etc.

Details Event Description : On this day our activity is to continue understanding
about domino effect and fault tree analysis. Mr Arnold
give a case and how to solve the case. The case given is
falling off the stairs. Then asked to analyze the case
from the loss of the hands or feet are cracked. The cause
fell from a ladder caused by spilled water. Spilled water
consists of several causes and we analyze the case in
detail until there is no possibility that it could occur in
the case.
Then we continue to understand the domino effect.
Domino 3 is an unsafe act or action such as improper
personal protective equipment, damaged equipment,
exposure to radiation, extreme temperatures, and so
forth.
Domino 4 is the existence of unplanned events such as
bumping, hit, falling from high altitude, pinched etc.
And the last is Domino 5 that is the occurrence of loss.
These losses have an impact on yourself, others,
society, companies, the environment and so on.

Title Event / Activity : Studying accident investigation and training on first aid
Details Event Description : The first activity I did was a discussion with Mr Arnold
about the practical work report. There are editoriaml
improvements in the report and some suggestions or
feedbacks received. After discussing the report, we
learned about accident investigation.
Which should be investigated is all incident or accident
should be investigated. And investigated purpose is to
find the cause, with the intention of preventing a
recurrence, rather than apportioning blame.
In this material there is single cause domino theory and
multiple causation.
Single cause domino theory, if one of the dominoes is
removed then the chain of events will be halted and the
accident will not happen. Element 3 is probably the
easiest factor to remove.
Multiple causation may be more than one cause, not
only in sequence, but occurring at the same time. In
accident investigation all causes must be identified.
Usually simple accident have a single cause. Major
disasters normally have multiple causes.
The categories unsafe act is: operating without
clearance, operating at unsafe speed, working on

Unsafe act can be active if worker deliberately removes

Our next activity is the training given by Kak Elin as
HSE Admin and Paramedic. Material presented on First
Aid. First Aid is an effort to provide immediate and
appropriate first aid to workers or other people who are
at work, who are sick or injured at work. The goal is to
maintain life and prevent death, prevent complications
that can arise from accidents, prevent the condition of
the victim getting worse, prevent actions that could
endanger the victim and protect the unconscious. The
principle of emergency action is: do not panic, the
location of the incident is safe, observe what happens,
find out how many casualties, whether that helps,
determine the main thing that threatens the victim's life
and call medical help.
Prinsp basic first aid is danger, response, air way,
breathing and circulation. The bottom line is knowing
what dangers happen, seeing the consciousness of the
victim, paying attention to the airway, see, hear and feel
the breathing, and notice the circulation of blood
covering the pulse and heart. If there is no response
from the victim then followed by CPR, given the
pressure on the chest as much as 30 times and artificial
breathing 2 times. CPR is dismissed when the victim
has a response, medical comes and tired (optimal).

Details Event Description : On this day, our activity is to observe MT Zaleha Ship
and Kandhaga Nusantara 1 Ship guided by Mr.
Anselmus Mab as K3L Inspector. The first ship we
observed was the MT Zaleha ship. On the vessel we saw
a spell box that serves to accommodate the tank so there
is no waste spill. MT Zaleha is a tugger ship, where a
tugger is a ship carrying oil. At this time the ship, we
found the welding cord and the cutting wires are
scattered. The cable is very dangerous because it can
cause workers to stumble and can cause a power zipper
that could endanger the environment. After we made an
observation on MT Zaleha Ship, we made an
observation to Kandhaga Nusantara Ship 1. This ship is
a cargo ship. Our activity is to see parts of the ship such
as cockpit, main deck, existing room like office, vip
room, abk (crew), captain room. Next we go to the
engine room part which is the core of the machining.
We are given an explanation by Mr. Jo as engineering
engine on the ship. We tell you about the parts that exist
and the function.
